We provide personalized care coordination by creating and managing individualized care plans that focus on the unique needs and strengths of each family, ensuring that all services and support are aligned to help them succeed.
Community Resource Navigation
Our team helps families access a wide range of community services, including mental health support, educational resources, and social services, making sure families are connected with the right resources to improve their well-being.
Counseling
As a Certified Addictions Practitioner, I also offer counseling services at The Counseling Coalition. This is a separate service aside from Wraparound. Call 307-257-2290 to get scheduled! Who We Serve (Wraparound)
- Diagnosis: A primary mental health or substance use disorder diagnosis, often defined by the DSM-5 criteria.
- Age range: Typically between 4 and 21 years old, depending on state regulations.
- Medicaid eligibility: Must be covered by Medicaid (There is a waiver for families not currently covered by Medicaid)
- Functional impairment: Significant challenges in multiple areas of life like academics, social interactions, or family functioning.
- System involvement: Current or past involvement with multiple support systems like child welfare, special education, or juvenile justice.
